Explore in-home evaluations →Testing helps address a specific clinical question. For example, resting and exercise oxygen measurements can support an evaluation of home oxygen needs when appropriate.
See how findings support care →We work with your primary care provider to understand the referral, provide an appropriate in-home evaluation or test, and share findings for the PCP’s review and follow-up planning.
How we work with your PCP →An in-home visit can give the referring PCP additional information to consider alongside the patient’s existing care. The evaluation or test is guided by the reason for referral and the patient’s needs.
The next step may be a PCP follow-up, further evaluation, or a care-plan discussion. Specific tests, services, and any equipment orders depend on clinical review and availability.
How we work with your PCP
Your primary care provider is central to this process. Apogee’s role is to support that relationship with in-home clinical information and coordinated communication.
We work with the referring PCP or care team to understand the requested evaluation or test and the information needed.
Our team reviews the request, discusses service availability, and coordinates the next step with the patient or caregiver.
The evaluation or testing provides findings for the PCP’s review, helping connect what is observed at home with the ongoing care plan.
The PCP can use those findings to guide follow-up, consider further evaluation, or discuss appropriate care and equipment needs.
Patients contacting Apogee directly can discuss their request and how to coordinate with their existing PCP. A request does not confirm an appointment or a specific service.
